This Agreement ensures you know what to expect so please read it thoroughly. Your above name in this agreement acts as a digital signature. Your IP address is collected with submission of this form.

I/We have read, understood and agree with the following conditions:

- a non-refundable booking fee of $100.00 is due at time of booking and subsequent invoice.
- payment of the remaining balance is due 14 days prior to the event.
- cancellation by the client less than 7 days before the date of the event will require payment in full.
- any children under 18 years of age, will be under the clients constant supervision during the shoot and that they are responsible for them at all times.
- standard turn around for processed images via online delivery will be 2 weeks post event.
- mail out of USB/printed images may be subject to a 4 week delivery.
- travel costs will be incurred by the client when Nic Hume Photography's travel is beyond 40kms from Silverdale NSW.
- In the highly unlikely event of cancellation by Nic Hume Photography due to injury, extreme illness, or an unavoidable total photographic failure that prevents Nic Hume Photography from photographing the event or delivering images, Nic Hume's liability shall be limited to a full refund of any booking fee or fees paid.
- this agreement supersedes all prior agreements. The only way to add or change this agreement is to do so in writing, signed by all parties.

I/We give permission to Nic Hume Photography to use any images taken during our session/event, for any form of advertising including web based promotions, brochures and general advertising. Please make a note in the Special Requests section if you do not wish to have images online.

I/We understand that full usage and printing of images is permitted upon full payment to Nic Hume Photography. I/We agree that all images supplied cannot be used for any commercial purpose and are strictly for personal use only.
